High-grade spondylolisthesis occurs when one vertebra slips forward by more than 50% over the vertebra below it, most commonly at the L5–S1 level.

✔️ Persistent low back pain that does not improve with conservative treatment
✔️ Pain radiating to the buttocks or legs (sciatica)
✔️ Numbness, tingling, or weakness in the legs

• Grade II: 26–50%
• Grade III: 51–75%
