Combined anterior and posterior approach for L4–S1 reposition, decompression, and fusion:
✅ ALIF at L5–S1
✅ TLIF at L4–L5
In this case, TLIF alone at L5–S1 would be not sufficient due to severe erosion of the S1 endplate and significant collapse of the L5–S1 disc height. A combined approach was therefore chosen to achieve optimal restoration of alignment and stability.
The postoperative result demonstrates:
✔️ Restoration of segmental lordosis (L4–S1)
✔️ Complete reduction of the L5 vertebral slip
✔️ Stable correction of Meyerding Grade I L5–S1 spondylolisthesis
A great example of tailoring the surgical strategy to the patient’s anatomy to achieve the best possible outcome.
